Hello Campers-to-Be and Parents! Please print, complete, sign, AND RETURN the following forms by the dates listed. Registration IS NOT COMPLETE until these forms have been received. All forms must be emailed to messiahwestcoast@gmail.com or snail-mailed to MWC, Attn: Chrissy Lopez, 41781 Rd. 142, Orosi, CA 93647.   1. Copy of Insurance Card (front and back) – Due July 1
  2. MWC Youth Contract – Due July 1
  3. MWC Parent Form – Due July 1
  4. Camp Oakhurst.Camper Health & Medical Release Form – Due July 1 (This form is specific to minors.)
  5. Camp Oakhurst.Food Allergy Form – Due July 1 (if applicable)
  6. Camp Oakhurst.COVID-19 FORM – Due Aug. 6 (On day of camp, print, sign, and take with you to camp!)
